The road runs from Kashgar over the Irkeshtam Pass to Osh in Kyrgyzstan, then through Batken and Isfara to Khujand in northern Tajikistan and on to Dushanbe.

Kulma Pass from China
The direct crossing from China, Karasu–Kulma, leads through Gorno-Badakhshan. It is a mountain road that operates seasonally.

Rail via Kazakhstan and Uzbekistan
Rail runs through Kazakhstan and Uzbekistan, then either north via Bekabad–Khujand or south via Termez towards Bokhtar and Dushanbe. The last leg is by road.
